Welcome to Restoration Health & Wellness. By accessing our website or using our services, you agree to be bound by the following Terms of Service. Please read them carefully. If you do not agree with these terms, you must not use our services.

1. Services Provided

Restoration Health & Wellness provides telemedicine-based health services, including but not limited to clinical evaluations, weight loss support, wellness optimization, and mental health services. Treatment plans may include behavioral recommendations, lifestyle support, lab testing, and prescription medications, when deemed medically appropriate by a licensed provider.

2. Eligibility

You must be at least 18 years of age to use our services. By registering for an account or booking an appointment, you confirm that you meet this eligibility requirement.

3. No Guarantee of Results

We make no claims or guarantees regarding specific outcomes, symptom improvement, or treatment success. All care is individualized, and results may vary from person to person. Statements on this website are for informational purposes only and do not constitute medical advice.

4. Provider-Patient Relationship

A provider-patient relationship is only established after you have completed a consultation with a licensed provider and the provider has accepted you as a patient. Receiving marketing emails, visiting our website, or submitting forms does not create a medical relationship.

5. Telehealth Consent

By using our services, you consent to receive medical care through telehealth. This may include video calls, phone calls, electronic messaging, and remote monitoring. You acknowledge the limitations of telehealth and accept the risks and benefits associated with this mode of care.

6. Payment Terms and Refund Policy

Restoration Health & Wellness operates on a direct-pay, fee-for-service model. All fees must be paid in full at the time of service unless otherwise arranged through a subscription or recurring payment agreement.

By clicking “Pay Now,” you authorize Restoration Health & Wellness, LLC to keep your credit card on file for future transactions related to ongoing services. This recurring charge is applied to cover the cost of your weight loss services and will remain in effect until the cost of services is paid in full or until you request a written cancellation.

You understand and agree to the following terms:

Charges will be billed on a monthly basis for the agreed-upon amount.
A surcharge may apply starting July 3, 2025, and each billing cycle thereafter.
You will receive a receipt for each transaction processed.
You will be notified at least 14 days in advance if the amount or date of any charge changes.
This authorization will remain in effect until canceled in writing by you.
You are responsible for submitting a written cancellation request if you wish to terminate recurring charges.

You also acknowledge that:

There are no guarantees regarding the results or outcomes of any medical treatment or therapy provided.
Restoration Health & Wellness, LLC is not responsible for individual results or performance.
No refunds will be issued for completed consultations, medications, or services, including in cases where specific results are not achieved.

7. Privacy and Data Security

We comply with the Health Insurance Portability and Accountability Act (HIPAA) and other applicable privacy laws. All personal and health data is stored securely and used only in accordance with our Privacy Policy.

8. Prescription Fulfillment

If a licensed provider determines that a prescription is medically appropriate for your care, your medication may be sent to a licensed U.S. pharmacy for fulfillment.

During times of national shortage, compounded medications may be prescribed if permitted under federal law. These compounded medications are prepared by licensed pharmacies in accordance with 503A regulations and state compounding laws. Compounded medications are not reviewed or approved by the FDA but may be legally prescribed during periods of FDA-declared drug shortages.

If compounded medications become unavailable, we will work with patients to ensure appropriate continuity of care, including transitioning to an FDA-approved option if available.

9. Changes to Services or Terms

We reserve the right to modify or discontinue any part of our services or these Terms of Service at any time, with or without notice. By continuing to use our services, you agree to the updated terms.
